A certificate of vaccination is not required. When importing pets, a veterinary certificate is required.

For importation of cats and dogs in Canada, you must provide a veterinary certificate of vaccination, obtained in the state veterinary hospital.

It should indicate breed, sex, age, pet name and registration number of animal, owner name and date of vaccinations.
